Passport Assistance
We provide passport appointment booking assistance for a hassle-free application of your passport. Our service includes passport consultancy, appointment scheduling and payment of fees.
Applicants are expected to appear at the DFA site on the day of their appointment to submit their requirements and have their photo taken,
All appointments are nonrefundable and non-transferable. Multiple appointments are not allowed.
FOR NEW APPLICANTS
✔️ Original PSA -issued Birth Certificate
✔️ PSA- issued Marriage cert, if married
✔️ One valid government -issued ID
FOR MINOR
✔️ Original PSA -issued Birth Certificate
✔️ School ID or certificate of enrolment , if applicable
✔️ Marriage Certificate of minor’s parents
✔️ Original and photocopy of valid passport of the parents

- UMID Card
- Driver’s License
- PRC ID
- Philippine Identification (Phil ID)
- Voter's ID

FOR RENEWAL
✔️ old/expired passport
✔️photocopy of the passport bio-page
FOR LOST PASSPORT
In case of a Lost ePassport - it will be treated as NEW APPLICATION.
✔️ Birth Certificate issued by PSA
✔️ PSA- issued Marriage cert, if married
✔️ Valid picture IDs
✔️ Affidavit of Loss in English (Original and photocopy)
✔️ Police Report in English (Original and photocopy) if lost passport is VALID
* If photocopy of the applicant's last issued passport is available, the Birth Certificate will not be required.
